A minor (under 18 years of age) : Complete and submit the Request for Access to Health Information form.An a dult (18 years of age or older) : The patient or their legally appropriate personal representative can access medical records with required documentation. Therefore, patient information may only be released upon receipt of a written request with appropriate patient authorization, valid subpoena, court order or as otherwise required by law. Any information contained in the medical record is confidential and protected by federal and state law. Northwell Health protects the confidentiality of patient medical records. Please note, a written request needs to be provided prior to processing and we are providing this downloadable form for your convenience.
